Merrimack Valley Community Partner (MVCP) / Long-Term Services and Supports (LTSS)
The Merrimack Valley Community Partner (MVCP) is a partnership between Elder Services of the Merrimack Valley (ESMV) and the Northeast Independent Living Program (NILP).
The MVCP has contracted with MassHealth to participate as a Long-Term Services and Supports (LTSS) Community Partner. The MVCP agencies provide over 40 years of combined experience in care planning and coordination as well as a broad knowledge of community services and benefits. We provide consumer choice and work to help support individuals to live successfully and independently in the community.
The MVCP has contracted with nine Accountable Care Organizations (ACOs) and two Managed Care Organizations (MCOs) to cover the greater Haverhill, Lawrence, and Lowell areas to
provide these services.
Once in the program, a Care Coordinator/Navigator will be assigned to work with you and your Accountable Care Organization (ACO) or Managed Care Organization (MCO), and Primary Care Physician (PCP) to create a care plan to support your goals.
The CP Navigator will help connect you with services and community resources to support your care plan, and provide coordination of services with other members of your care team. Participation in a LTSS CP program will not affect other services you receive.
Who is Eligible?
- Must be on MassHealth
- Between the ages of 3 and 64
- Has a need for Long-Term Services and Supports
