Merrimack Valley Community Partnership (MVCP)

Long-Term Services and Supports Community Partners (LTSS CP) is a program offered by MassHealth to help members find local services and benefits to improve their independence and wellbeing. In 2018, NILP and AgeSpan partnered to form the Merrimack Valley Community Partnership (MVCP) to provide long-term services and supports to the aging and disability populations. 

We serve individuals enrolled in 11 Accountable Care Organizations (ACOs) that cover the greater Haverhill, Lawrence, and Lowell areas. We connect eligible members with services and community resources to support their care plan and provide coordination of services with other members of their care team, which may include ACO clinical staff members and Primary Care Physicians.

Our team of Care Coordinators called Navigators assist individuals in achieving a range of goals, including setting up Long-Term Services & Supports (LTSS) in their home, guardianship, citizenship, housing, financial stability, and other obstacles related to the social determinants of health. 

Who is Eligible?

  • Must be on MassHealth
  • Between the ages of 3 and 64
  • Has a need for Long-Term Services and Supports

Long-Term Services Coordination for One Care Members

NILP provides Long-Term Service (LTS) Coordination for three One Care plans in Massachusetts, Commonwealth Care Alliance, Tufts Unify, and UnitedHealthCare. Members enrolled in One Care are persons who are dually eligible for both MassHealth and Medicare. 

Every One Care member has the right to have an LTS Coordinator on their care team. LTS Coordinators serve as both an independent advocate for the member and as a resource to learn about long-term service options they might be eligible for in the community. 

You can ask for an LTS Coordinator at any time. Tell your Care Coordinator in your One Care plan and they will provide you with more information. Our LTS Coordinators are experts in areas such as independent living, recovery, and aging.
